On 29/10/08 the Department of Health published some draft standard forms for supervisory bodies. On 14/11/08 they published draft forms for managing authorities.

Draft deprivation of liberty safeguards standard forms are being made available now in order to facilitate best practice, training and preparation in advance of April 2009, when the deprivation of liberty safeguards come into effect.
The forms can be adapted but, as drafted, they help all those involved in the safeguards processes to fully meet the requirements of the deprivation of liberty safeguards legislation.
